This website provides information about Fusion Rehab and Wellness, its clinics, services, and programs, and lets you request an appointment or contact a clinic. Information on this site is general and does not replace an evaluation by a licensed clinician.

Appointment requests

Submitting an appointment request does not reserve or confirm an appointment. A clinic team member will contact you to discuss scheduling. Please do not include medical details in the form.

Online shop

Orders placed through the Fusion shop are fulfilled by the Fusion team. Prices, availability, shipping, and returns are described on the Shipping & Returns page and at checkout. An order is confirmed only after payment has been verified.

Content and trademarks

Text, images, and the Fusion Rehab and Wellness name and logo are the property of Fusion or its licensors and may not be reused without permission.
